Where to Find Student Forms

Student forms are kept in the Student Services office (ISAT paper and pen graphic122). Some forms may also be found on the third floor HHS suite where the faculty offices are located. You may also visit Registration Services in Warren Hall, Room 300.

GS internship forms are located in HHS 3234. Talk to your GS advisor prior to taking a form.

Remember to be aware of important dates and deadlines. Check the JMU calendar or ask Student Services if you have any questions.
